Mind, Body and Soul surveys

Healthwatch Rotherham are undertaking a new project titled 'Mind, Body and Soul' over the next few months to find out what people in Rotherham want from services. We will be releasing three surveys in total.

The first survey is on your lockdown experience and you can complete this here: https://forms.gle/XdLcqLJcg9pGbLeT8

The surveys are only short, however if you find it easier to discuss your responses over the phone directly, rather than filling it out yourself, please contact us with your contact number and name, and Healthwatch will get back to you to fill out the surveys with our help.
