Joining up our Intermediate Care and Reablement Pathways Workshop

Rotherham health and social care partners are working together to join up our intermediate care and reablement services to help support people at home when they might otherwise be in hospital or help them return home quicker after being in hospital.

Evidence shows people do much better at home and people tell us they want to be at home and as independent as possible.
