About the communication card

Purpose
The card is a simple, practical way for people to tell health and care staff how they need information and support. It follows the Accessible Information Standard so appointments are easier to access for everyone.

Co‑production
We worked directly with people with sight loss in Rotherham to design the card. Their feedback shaped the final layout, wording and format so it’s works at all appointments.
